We will bill your visit directly to your insurance provider if you supply an updated proof of insurance at each visit. Please check with us to see if our practice is included in your health insurance network.
We are happy to file the claim for you, but the balance is your responsibility if your insurance company or plan does not pay after 30 days. Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us.
We accept Medicare patients. As a courtesy, we will also bill your secondary insurance provider directly. You will be asked to sign an Advanced Beneficiary Notice for services not covered under Medicare. You shall be accountable for covering these services.

Jana and I are retiring to the mountains of New Mexico! We will be closing the office for good on June 30, 2020 as our lease has expired. Thank all of you for 29 great years.

Expect me to call you for a phone interview prior to authorizing your refill. I will likely give you a 6 month refill on most routine meds. Calls may
be in the evening hours and and may not be from our main office phone #.
Have pertinent information readily available such as pharmacy phone #, meds you want refilled and insurance information as needed. If you can provide BP readings or other health info, that helps.
We are billing telephone visits to your insurance as recommended and allowed. Cash televisits are available.
